The Underworld is the whole system of connections, smuggling, and shady dealings which eludes the World Government's control. It is mainly set in the New World. The Underworld is made of most of the criminal organizations from all over the world. Influential people such as Yonko, Shichibukai, World Nobles or Supernovas might be involved in it, as it allows for a lot of slave trading, weapons smuggling and goods dealing. The Underworld also possesses a lot of mercenaries and hitmen for hire. Most of its activity takes place in the New World, the only sea not under the direct dominance of the World Government, although its also present in other seas. The most influential man in the Underworld was previously said to be Donquixote Doflamingo under the alias Joker. Other important figures are the Emperors of the Underworld, who are often the leaders in their specific activities. Mother Carmel is a mysterious person associated with Charlotte Linlin. Details about her are unknown; Capone Bege and Charlotte Chiffon guessed that she might be a benefactor of Big Mom that went missing. During every tea party, Big Mom places a picture of Carmel across from her at the table. If Carmel's picture is damaged, Big Mom will enter a psychological crisis, which will render her able to be physically injured. In her portrait, Carmel is shown as an older woman with a large face and a square-shaped jawline. She has wavy light hair and wears lipstick. She wears dark robes similarly to nuns like Miss Catherina. She was seen holding a cigarette in her hand. Trivia Carmel's name may be a reference to the Carmelites, a religious order known for selling sweets in Japan, which corresponds with Carmel's nun-like outfit and title of "Mother". The sweets connection also matches the Big Mom Pirates and Charlotte Family food theme.

Charlotte Katakuri (DogTooth) is the second son of the Charlotte Family and one of the Three Sweet Commanders of the Big Mom Pirates. Katakuri is a large, muscular man with short spiky black hair and a scar on the left side of his face. He wears a massive light ragged scarf that covers his mouth and reaches his shoulders. He also wears a torn leather jacket that exposes most of his torso, dark gloves and pants, a belt with a skull-shaped buckle, and spiked boots with spurs. His torso is covered in tattoos, and he wears spiked bracelets on his arms as well as knee pads with a single spike on them. Katakuri appears to have a very serious, no-nonsense demeanor, seeming to care greatly about his crew and family's objectives. He is not afraid to take initiative even if it may be out of turn for him, as shown when he quickly dispatched Jigra after foreseeing his assassination attempt on his mother.
